As I have attained age of 18 years as on ____________________, I request you to change my status in the Folio and remove the Guardian Name and also update the following details in your records:
Documents to be submitted:

1. Self attested PAN & KYC acknowledgment copy of the major [Mandatory].

2. Age Proof – Birth Certificate / School Leaving Certificate/Mark Sheet issued by Higher secondary

school of respective states, ICSE, CBSE etc. / Passport / PAN Card / Any other suitable proof issued by Government Authorities.

3. For updation of Bank details (any one of the following):

4.  Cancelled Cheque with Name & Account number printed on it.

5. Copy of the Bank Pass book / Bank Statement showing A/c holder Name and A/c No., duly attested by the Bank Manager where the above mentioned bank account is maintained.

Note: The date of the letter & KYC acknowledgement should be on or after the date of attaining the age of majority. You may produce the original documents mentioned above along with the photocopies at any SBIMF / CAMS Service centre and the originals shall be returned to you after verification.
